I think my biggest pet peeve in Guild Wars are just people with no common sense at all.
A few examples:
You're about to do Duncan on HM with a PuG. The group looks fine, and after repeatedly saying that we cannot get in if people have not done all four bosses, the door will not open. So we fight our way there in Hard Mode as not to die running, and when we get there nearly 20 minutes later, low and behold, the door is shut. Bravo. We tell the person responsable to just leave so we can get a new person. No one leaves.
Say you're selling an item in Kamadan, and you advertise it as a very nice item. With this in mind, some random person comes up to you and gives you the kingly offer in the Trade Screen of a worthless green, 130g, and a yellow and a blue dye. You decline, tell him it's not in his price range, he curses at you.
Say you're in the same situation, and in the trade chat, someone begins to spam guild recruitment. *This annoys me to no end*. They go to LA D1 and they recruit in the dang trade chat to get the attention of the sellers. Local chat is there for a reason.....
And little things people do, like argue in leet speak with multiple swear words per sentance when you can easily tell neither party is over seven years of age, or adults that think everyone under 18 should be banned because of the immature stereotype, or people that manage to "Agro Anyway".
I'm done here, too many to list.

To be fair, this can be really confusing. It's not enough to have "done all four bosses" - the first time I killed Duncan, I got the reward afterwards and figured, hey, now that I've killed all four bosses I can just repeat Duncan a bit. So I joined another Duncan group and we get to Slavers and the door won't open - and it's my fault. I figured, well, it must be because I got the reward last time so the four bosses aren't crossed out in my quest log.
I did the four bosses again, and then I killed Duncan again, and then I figured that I'd wait to accept the quest reward so that I could go back to Slavers and kill Duncan a few more times. Lo and behold, I went back to Slavers and the door was closed - because even if I'd killed all four bosses, I had already killed Duncan.
So actually, the requirement is, "have killed all four bosses, since the last time you killed Duncan."
I kind of wish this was clearer because after the first time I checked the Wiki and didn't find an explicit explanation of who can and cannot kill Duncan again. Apparently it's very similar to the DoA, but I don't do the DoA so that didn't ring a bell.
I admit that if you're doing HM you should have already done the trial and error to figure this out, but who knows how many other confusing conditions there are. It's more complicated than just, "OK, who hasn't killed all four bosses?"

People who don't at least say "Thank you" or "thnx" or "tyvm" or "ty" after a trade.
Mo/E's that have half of their skills dedicated to Fire Magic and say that the skills are there to "protect himself"
Assassins that say "I'll protect you" to players such as the Mo/E above..
The list could go on and on..

people who tell others to 'hit alt f4 for the new xxx' that's just plain annoying.
there are many new people playing the game, telling them this if very elementary schoolish.
Like calling people names, that is for kids---not the age of the people who are supposed to be allowed to play this game.
And if you are an adult calling people nubs, noobs, etc...SHAME ON YOU! (caps intentional)----grow up and act your age, please.

1) players who are rude
2) Players who cheat other players
3) bringing race, religions and politics into the game, anytime i see that, local chat will be off.
4) this often happens in town, players who, when other players ask a question, no one answers, when someone try to answers them at best they can, people start bashing them, saying their answers are wrong bla bla bla .... so why did no one answers when the question was first ask? why bash the players who try to help.

Loot scaling.
People with their plx, u, lol, Q, tombs, inscriptable and other stupid things.
It taking at least an hour of spamming to sell something.
All the "lol 200g kk"-whispers when trying to sell something.
People only wanting req 9 and lower stuff. Shields, OK I understand, but with weapons, if it's your primary attribute you'll have 14+ in it anyway, so why demaning only req9?
HA elitism, how can a new player get rank 3 if < rank 3's are not allowed to join teams that stand any chance of winning...
No modest female paragon armor.
Obsidian armor. For most classes, it's ugly. I wouldn't wear it even if I got it free.
Black dye. Looks horrible on most armors.

*Slow sales are normal. No one's gonna fall to your feet everytime you post something up for sale.
*Req7-9 is the best. Who wants less Req #? Common sense.
*Elitism is in fact wrong and biased. You are absolutely right on that one. The only way to bypass that is to find an unbiased group and prove to them your skills. An infinitely better and rather obvious choice is HA with your guild (if you have one). If you don't, take the time and search around.
*Obsidian Armors: Some are in fact ugly. But I find some to be good, too. I have my assassin wearing FoW and it fits her theme nicely!
Psylocke Deadpool.JPG
*Black Dye: C'mon now... that's sooo untrue! You just gotta do your homework and plan ahead at finding the armors that do look good with black. Not everything looks good or bad with black, and the option IS there at your disposal. Freedom of choice!

any kind of "leet speak"
It's fine if it's said in jest and cynicism. I just don't like it when it's a part of someone's normal internet vocabulary. I don't mind misspellings and bad grammar, that's understandable, but these things are deliberate mistypes and net shorthand that is just plain irritating.

Oh yeah this one for sure. I can't stand the effect technology has had on the English language. I asked a kid one time." Why do you type like that?" His reply was." Everyone does and it's cool." Cool? Cool?! If ignorance is cool then mental retardation must be the most awesome "leetest" thing ever.
I also hate people with wooden modems. Honestly if you still use dial up then you probably should not be playing an online game that isn't Flash animated. It's the information age people. Pony up that extra cash and get with the broadband or if mom and dad won't pay for it then tell them you'll cut yourself or whatever it is you kids do to get your way these days.
I also hate people in town who won't answer a simple question. Instead you always see."Go look it up on wiki noob." When nine out of ten times it would have been quicker to just type the answer.
Finally I hate people that quit mid mission because one non essential party member dropped. "OH NOES! TEH RIT QUIT I R QUITTING TOO!!11!!1!!" Yeah that bothers me.

Srsly (OMG, seriously*). It is not "leet speak", it is called abbreviations. I bolded a word in your post. That is also an abbreviation. People who get off on one because someone said "lol" pisses me off. Choosing to type / speak with abbreviations doesn't show you have a lack of proper grammar imo <-- did it again

In no particular order...
-Leavers
- When someome pm you and then when you are going to awnser it says that he/she is offline.
-People tho think they are the elitist in all, specially in pvp but in pve too, and calls everyone ''noobs''
- Leet speak, its very dificuilt for me to understand a word of it because im not a native english speaker.
- Spam, with the trades, guilds, and the 'lfg' thing.
- Beggars
- People who asks for help and then they dont even say ''thanks''
- When you are selling something then someone comes and offer you items but not the gold you want for it.
- Seeing lots of people using black dye, just beacuase it is the most expensive dye it does not mean that it looks that great in every armor.
- Obisidan armor, it looks cool in some classes but in other classes it looks just ugly.
- (in this one i'm not triying to insult anyone, its just my point of view) I just hate when i see most of eles are always all in RED, with that RED hair (that in my opinion looks ugly and fake), and RED dyed armor, and i hate it specially when i see those all RED eles have one of the words ''Fire, Flame, Burning etc...'' in their names, comon eles are not just fire.
- People who thinks that some classes are usseless like assassins, mesmers, paragons... comon every class is useful for something.
I may have more pet peeves but i'm not going to post them all.

This is my pet peeve.
Also when people get annoyed when I /rank in PvP arenas.
Sure, it's childish. But....just because I have one, and you don't...does this mean you should get bitter and fill my text log with childish threats and insults?
People telling me that Paragons and mesmers suck is up on my pet peeves list too.
